Through Telehomecare, patients gain the skill and confidence to efffectively manage their condition at home.
In early results from the first 130 patients enrolled by Willian Osler Health System, (Central West LHIN Telehomecare Program) Telehomecare demonstrated a reduction of 43% in ER visits and a reduction of 71% in in-patient admissions compared with the pre-Telehomecare rate.

Patient Care Delivery
Telehomecare complements the care you are already providing to your patients. Telehomecare nurses will keep you informed of your patient's progress on a schedule you prefer.
Nurses complete a discharge assessment and set a maintenance plan linking your patient with community resources. A final report is sent to you and your patient's circle of care.
